Teeth Whitening: Professional vs. At-Home
By Dr. Harut Hakobyan, Olive Dental Studio · Burbank, CA
Comparing professional in-office whitening with at-home kits. Which option delivers the best results for your smile?
Teeth whitening is the most common cosmetic dental procedure in the country — and also the most confusing one for patients to navigate, because the options range from $20 drugstore strips to $800 in-office sessions. This is a clear comparison of what actually works, what doesn't, and how to decide.
How whitening actually works
Almost all real whitening (the kind that lightens the actual color of your teeth, not just polishes off surface stains) uses some form of peroxide — either hydrogen peroxide or carbamide peroxide, which breaks down into hydrogen peroxide on contact with your teeth. The peroxide diffuses into the tooth and bleaches the pigments inside. Higher concentrations work faster; lower concentrations work more gradually.
Whitening toothpastes that don't contain peroxide work by mechanical polishing — useful for surface stains from coffee or tea, but they can't change the actual color of your teeth.
Drugstore strips: what they can and can't do
Drugstore whitening strips (like Crest 3D White) typically use 5–10% hydrogen peroxide. They're affordable and effective for mild yellowing, especially in younger patients with minimally porous enamel. Limitations:
- The strips don't conform perfectly to your teeth, so coverage is uneven (corners and edges often lighten less).
- Concentration is limited by what's safe for unsupervised use.
- Treatment takes 2–3 weeks, with daily 30-minute applications.
- Sensitivity is common during use.
Custom take-home trays
This is the option we most often recommend for patients who want professional results without the cost of in-office whitening. We take impressions of your teeth, fabricate custom trays, and provide professional-strength gel (usually 10–22% carbamide peroxide). You wear the trays for 30–60 minutes a day, or overnight if your enamel tolerates it, for 1–2 weeks.
Why custom trays beat drugstore strips:
- Custom-fit trays mean even, predictable results across every tooth surface.
- Stronger gel concentration means faster, more dramatic results.
- You can re-use the trays and order touch-up gel any time. Most patients touch up once a year.
- Sensitivity is more manageable because we can adjust the concentration to your tolerance.
In-office whitening
For patients who want results immediately — say, before a wedding or photo shoot — in-office whitening is the fastest option. We isolate your gums, apply a high-concentration peroxide gel (often 25–40%), and activate it with light or heat. One session takes about an hour and produces several shades of lightening on the spot.
It's more expensive than take-home trays and the result, on its own, isn't always longer-lasting. Most patients who choose in-office whitening also leave with custom take-home trays for maintenance.
What whitening can't fix
Whitening only works on natural tooth structure. It will not change the color of:
- Existing crowns, veneers, or bonded fillings (these stay the same color while the rest of the tooth lightens — sometimes producing a mismatch you'll want to address)
- Deep, intrinsic stains caused by certain medications (like tetracycline)
- Severely worn enamel exposing yellow dentin underneath
For those cases, veneers or bonding may be the better path. We'll tell you honestly which approach makes sense at your consultation.
Sensitivity and safety
Tooth sensitivity is the most common side effect of whitening — usually temporary, lasting a day or two after a session. It's manageable with a sensitivity toothpaste (like Sensodyne) for a week before and during treatment. We adjust concentrations and frequency based on how your teeth respond.
Whitening is safe when done correctly, but it's not for pregnant or nursing patients, kids under 14, or anyone with active cavities (those need to be treated first).
Our recommendation
For most patients, custom take-home trays from a dentist offer the best combination of result, cost, and longevity. In-office whitening is great for fast results, and drugstore strips can be a reasonable starter option for very mild discoloration.
